Job Summary
The Lead Care Manager is responsible for assisting members and their families in obtaining and understanding services and programs available through the CalAIM program in order to improve their health and overall well-being.
Responsibilities/ Job Duties:
Serve as a
dedicated care coordinator
for enrolled members, ensuring they receive
personalized, whole-person care
. Conduct
in-home visits (majority of the time)
as well as visits to hospitals, shelters, or clinics to assess patient needs. Provide
clinical support
and health-related education to patients based on their specific conditions and care plans. Collaborate with community resources, healthcare providers, and insurance teams to facilitate access to necessary medical and social services. Utilize
telehealth services
when necessary to accommodate patient preferences or scheduling constraints. Address
social determinants of health
, working closely with patients to overcome barriers to care such as transportation, food insecurity, and housing instability. Ensure proper documentation and maintain patient records according to compliance standards. Work alongside
Community Health Workers (CHWs)
, who focus on
patient enrollment
, while the
Lead Care Manager handles direct patient care and coordination
. Minimum Education and Experience Qualification Requirements: Medical Assistant Diploma or CNA
Schedule/ Shift: Monday - Friday 8am - 3pm
