Facility Liaison
to support and help lead our Social Services Department. If you're passionate about resident-centered care and enjoy building meaningful relationships with families and care teams, this is the role for you.
What You'll Do: Support the Social Services Director in coordinating care plans and discharge planning Serve as a key point of contact for residents and their families Help ensure smooth transitions throughout the care continuum Advocate for resident needs and assist with emotional and psychosocial support Collaborate with nursing, therapy, and administrative teams to promote positive outcomes What We're Looking For:
Experience in a skilled nursing or healthcare setting preferred Strong communication, interpersonal, and problem-solving skills Compassionate, empathetic, and able to navigate sensitive conversations Organized and able to manage multiple priorities with care and professionalism Why Ridgeway Post Acute?
