AdventHealth Ocala Located in the Horse Capital of the World, home to the World Equestrian Center, with outdoor activities such as golf, kayaking, horseback riding, nature trails, a vibrant downtown, family parks, and spectacular springs. The role youll contribute: The Social Work Care Manager intervenes with patients with complex psychosocial needs, assists with eligibility for social programs, funding sources, community assistance, and facilitates transitions of care or discharge planning. Offers crisis intervention, develops discharge plans for high-risk populations, and collaborates with interdisciplinary team members to ensure patient-centered care. Ensures efficient care, monitors resources, and manages patient flow, safety, satisfaction, readmission prevention, and length of stay. The value youll bring to the team: Psychosocial assessment and interventions, including evaluating risk factors, supporting coping with life transitions, and mobilizing resources. Providing emotional, social, and financial support, grief counseling, crisis intervention, and advocating for patient empowerment. Reviewing patient information and incorporating patient/family goals into care planning.
