In this pivotal role, you will maintain the delivery of quality care by implementing and adhering to individualized nursing care plans. You will also supervise Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs), ensuring a collaborative environment focused on patient-centered care. Key Responsibilities: - Conduct comprehensive nursing assessments for all patients within 8 hours of admission, using the data to create tailored care plans.- Identify patient and family educational needs from initial assessments and collaborate with multidisciplinary teams to meet those needs.- Provide timely and accurate patient information for team conferences, ensuring ongoing evaluation of care plans.- Participate in the patient rehabilitation program, prioritizing direct patient care using the nursing process: assessing, planning, implementing, and evaluating care.- Initiate discharge planning from the day of admission and lead patient and family education relevant to discharge goals.- Ensure effective communication among care providers regarding changes in patient status to maintain continuity of care.- Mentor and supervise nursing support staff, utilizing effective problem-solving and organizational skills to manage daily activities.- Foster a safe patient environment by adhering to safety protocols and identifying potential hazards.- Actively contribute to team-building efforts and create a positive work atmosphere.- Maintain open communication with nursing management regarding clinical supplies, operational needs, and training requirements.- Assume charge responsibility as requested and pursue continuous professional development through ongoing education and self-study.- Perform additional duties as required.
