Job Responsibilities- Conduct history and physical assessments on new participants and perform periodic reassessments.
- Interview and evaluate potential PACE applicants and their caregivers, contributing to the intake and assessment process.
- Engage in discussions regarding each participant's medical conditions and future care planning with them and their caregivers.
- Execute assignments collaboratively across all divisions within the organization.
- Collaborate on nursing treatment programs for participants.
- Help establish and maintain operational goals as a key member of the leadership team.
- Contribute actively to interdisciplinary team meetings, providing education on diagnoses and treatment options.
- Participate in family conferences and decision-making processes.
- Educate participants and families on health maintenance and chronic conditions, in cooperation with nursing staff.
- Oversee the healthcare and treatment programs designed by the interdisciplinary team.
- Conduct physical examinations, order and evaluate diagnostic tests, and document results systematically.
- Participate in the on-call rotation as required.
- Current and active plenary physician license to practice in Florida - REQUIRED
- Board Certification in Internal Medicine or Family Practice - REQUIRED . Board Certification in Geriatrics preferred.
- Experience or additional qualifications in geriatrics - Preferred
- CDS Certification and DEA Registration - REQUIRED
- Previous supervisory experience - REQUIRED
- Minimum of one (1) year of documented experience with a frail or elderly population.
- Strong skills in physical assessment and chronic disease management for older adults.
- A desire to work in an innovative healthcare setting.
- Able to adapt to changing priorities in accordance with participant needs.
- Active physician license in Florida
- Board Certification in Internal Medicine or Family Practice
- CDS Certification
- DEA Registration
