The primary responsibility of this position is to grow the agency's client base within your assigned territory. You will be tasked to promote our high-quality healthcare services (Palliative and Hospice) to prospective referral partners while providing account management to existing partners. Meets and exceeds territory growth targets. Ability to travel independently in the community to attend meetings with referral sources and the public. Demonstrates ability to develop basic clinical knowledge to support and present KeystoneCare's marketing activities and services. Manages existing accounts to ensure continued and deepening partnership. Coordinates post-acute discharge for homecare and hospice with case managers, discharge planners, social workers, and physicians to provide a seamless transition to home or SNF. Regularly provides update to management team on prior period activities and upcoming marketing plan. Ability to communicate at all levels with the healthcare community. Develops and participates in Community education and outreach events.
