Manage acute and chronic conditions
Develop personalized care plans
Integrate and work closely with the IDT
Partner and work collaboratively with physicians
Compensation & Benefits
Compensation is fee for service (FFS); expected compensation is estimated salary range of $109,936 to $131,926 for full-time and $26,139 to $31,367 for part-time annually with no cap on productivity income potential with full benefits.
