APP - Primary Care in Cheshire, CT
Certified APRN (Family or Adult/Geriatric) or Physician Assistant

Position Overview:

We are seeking a compassionate and motivated Nurse Practitioner or Physician Assistant to join an outpatient adult medicine practice. This centrally located clinic offers convenient access to a wide range of services, supported by a welcoming team and a patient-centered approach to care.
Shift Detail: Monday Friday

Role Details:
Position: Nurse Practitioner (Family or Adult/Geriatric APRN) or Physician Assistant (PA)
Setting: 100% Outpatient Primary Care Adult patients (18+)
Schedule: Full-time, weekdays only
Call: No call required
Provider Support & Development:
Strong administrative and clinical support for advanced practice providers
Modern EHR and informatics systems that enhance care quality and provider satisfaction
Access to continuing education resources, learning platforms, CME, and leadership development programs
Structured mentorship and orientation for new hires and providers new to practice
Scope of Practice:
In accordance with established protocols and in collaboration with supervising physicians, Nurse Practitioners and Physician Assistants will:
Perform routine physical examinations and assessments
Develop and implement individualized medical, educational, and psychosocial treatment plans
Coordinate additional evaluation or referrals to specialists as needed
Deliver care within scope and refer appropriately for conditions outside of their scope of practice
Qualifications:
Licensure: Eligible for or currently holding an unrestricted medical license in the State of Connecticut
Training: Certified APRN (Family or Adult/Geriatric) or Physician Assistant
