About the Spanish Speaking Case Manager Role
As a Case Manager within our Long Term Services & Supports (LTSS) team, you'll play a vital role in supporting adults with physical disabilities to access the care and services they need to live with dignity-whether in a nursing facility or at home. This position is ideal for someone who is compassionate, organized, and comfortable navigating complex systems with empathy and clarity.
Starting Rate: $20.48/hour, per the current union agreement
Spanish Speaking Case Manager Core Responsibilities
  • Deliver culturally responsive, trauma-informed, person-centered care
  • Conduct comprehensive assessments and develop individualized service plans
  • Link individuals to authorized services and community supports
  • Monitor participant satisfaction, safety, and adherence to service plans
  • Ensure Medicaid eligibility is maintained and assist with resolving related issues
  • Maintain accurate and timely documentation
  • Collaborate with internal and external stakeholders to coordinate care
  • Provide crisis intervention and support when necessary
What You Bring
  • Bachelor's degree in social work, psychology, or a related human services field OR
  • Three years of experience in a social service or healthcare-related setting OR
  • Current Registered Nurse (RN)
  • Ability to speak and write in Spanish
  • Strong interpersonal and critical thinking skills
  • Comfort with electronic documentation and remote communication
  • A valid driver's license and reliable transportation

This position is part of a collective bargaining unit and follows union-negotiated terms for compensation and benefits.
Location & Schedule
This position is based in Luzerne County, PA. Work is primarily community-based and includes travel to homes and nursing facilities.
Office hours are Monday through Friday, 8:30 AM to 4:30 PM.
