Description Conduct thorough evaluations for patients in both inpatient and outpatient settings and develop personalized treatment plans. Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function. Collaborate effectively with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes. Provide education to patients and families regarding therapeutic objectives, discharge planning, and recommended home exercises. Maintain accurate documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory standards. Actively engage in care team rounds and interdisciplinary conferences to ensure seamless and coordinated care. Education
Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution. Certifications
Active state licensure as a
Physical Therapist CPR certification desired Specialty certifications (such as neurological or geriatric) are advantageous but not mandatory Skills
Prior experience strongly favored Ability to address complex medical and mobility challenges Exceptional communication and teamwork abilities Proficient with digital charting and clinical documentation systems Benefits
