This role requires proficiency in both English and Spanish. As a vital part of the Children's Long Term Support (CLTS) Waiver Program, you will help families with children who have developmental, physical, or severe emotional disabilities access necessary services to thrive in their homes and communities. This full-time position operates Monday through Friday during standard first shift hours, with flexibility to meet the needs of families. Remote work is available after a defined period! Being bilingual in Spanish will increase your hourly pay to $24! Key Responsibilities: Conduct assessments of functional abilities and client needs. Develop and implement personalized service plans in collaboration with families and service providers. Facilitate care and service coordination while maintaining efficient communication among team members. Maintain accurate documentation and client records within established timelines. Understand and apply casework principles, community resources, and human behavior dynamics. Participate in staff development programs and clinical supervision as required.
