Join Our Team as a Pediatric Care Manager!
As a Pediatric Care Manager (RN/LICSW), you will play a vital role in empowering our members to achieve their optimal health through effective care management and coordination services. You will work closely with members, caregivers, and healthcare providers to develop personalized care plans that enhance overall health and well-being.
In this role, you will:
- Engage with members through face-to-face or telephone assessments to identify care coordination needs.
- Conduct thorough, person-centered evaluations covering health history, mental health, social factors, and necessary supports.
- Coordinate a range of services, including physical and behavioral health care.
- Manage medication regimens effectively to ensure adherence and reconciliation.
- Identify challenges to effective care and implement appropriate interventions.
- Create tailored care plans focused on overcoming barriers and achieving health goals.
- Set clear priorities and goals with time frames, and share progress with members and their families.
- Continuously monitor member progress, schedule follow-up communications, and facilitate referrals.
- Collaborate with Care Coordinators, Community Health Navigators, and other providers to optimize member care.
- Participate in regular meetings with the DC Strong Start Early Intervention Program to enhance service delivery.
What You Need:
- For RN candidates: A current and unrestricted Registered Nurse license in Washington, DC, with an Associate's Degree in Nursing (Bachelor's Degree preferred).
- For LICSW candidates: A current and unrestricted Licensed Independent Clinical Social Worker (LICSW) license in Washington, DC, along with a Master's Degree in Social Work.
- At least 3 years of experience as an RN or LICSW, particularly with pediatric patients in acute care, home care, or community health settings.
- Preferred: 1 year of case management experience supporting Medicaid members or pediatric populations in managed care organizations.
License Requirements:
- Current, unrestricted RN or LICSW licensure in Washington, DC.
- A valid driver’s license and car insurance.
