Provide care in patients’ homes and virtually, aligned with your licensure and certifications. Diagnose and treat acute and chronic conditions such as diabetes, hypertension, kidney disease, and others, including injuries and infections. Order and interpret diagnostic tests like labs, x-rays, and EKGs. Prescribe pharmacological and non-pharmacological treatments. Perform procedures such as IV placements, wound debridement, joint injections, and vaccinations. Collaborate with physicians, specialists, and the healthcare team to develop and evaluate care plans. Manage patient care plans, discharge planning, and visit schedules. Communicate effectively with patients and families about diagnoses and treatment. Oversee and manage a census of longitudinal care patients, transitioning appropriate patients to hospice or back to usual care as needed. Perform care transition evaluations and acute visits when necessary. Maintain accurate patient records and documentation. Educate patients and families on disease management and quality of life. Base salary plus incentive bonus. Participation in the Optum Care Advanced Provider Clinician Partnership Plan. 401k with matching and stock purchase programs. 160 hours PTO, 40 hours CME, and 9 holidays annually. Comprehensive benefits package. Requirements include graduation from an accredited Nurse Practitioner or Physician Assistant program, current licensure and prescriptive authority, comfort with chronic disease management in Medicare-aged populations, a valid driver’s license, and willingness to travel at least 25% with overnight stays. Preferred qualifications include 2+ years of internal medicine or family practice with geriatric care, home health experience, knowledge of palliative care, bilingual skills, and the ability to work independently and with teams.
