is seeking an experienced
Case Manager
for its congregate program at
Prospect Residence in the Bronx . The main responsibility is to provide supportive services to program participants in a housing setting, including assistance with personal, vocational, and educational goals, using a person-centered / Harm Reduction model to support recovery, hopes, and opportunities. Monday to Friday, 9am - 5pm Essential Duties
Ensure clients receive effective case management services. Conduct assessments and develop service plans with mutually agreed goals. Perform face-to-face contacts, including home and office visits, with a minimum of one of each per week. Assist clients with medical care access, medication adherence, daily activities, nutrition, recreation, employment, and other services to promote independent living. Reassess and review service plans quarterly to meet clients' evolving needs. Ensure each client has psychosocial support. Provide crisis intervention as needed. Monitor clients’ medical conditions and adjust services accordingly. Arrange support and home health services, making appropriate referrals. Offer counseling and guidance on tenant rights to maintain housing. Assist with moving from hospitals or nursing homes to apartments, including transportation. Collaborate with vocational and entitlement specialists for advocacy and referrals. Help clients find alternative housing if necessary for health reasons. Coordinate all supportive services required. Work collaboratively with all involved providers. Build trusting relationships with clients through regular contact. Maintain detailed documentation of all contacts and services provided. Participate in weekly supervision and submit schedules of field visits. Engage in training, seminars, case presentations, and meetings. Qualifications
Education and Experience : Bachelor's Degree required, with at least two years of field experience and direct work with mental illness. Experience in counseling individuals, families, and groups, especially with mentally ill populations. Bilingual in Spanish and bicultural preferred, especially with homeless populations.
