Employees may perform other related duties at an equivalent level. Not all duties are necessarily performed by each individual. Conduct community outreach and health education activities supporting CHS program goals, focusing on priority populations to achieve equitable health outcomes. Provide education, outreach, and support in areas such as: Client navigation and referrals to programs serving priority populations, including: Providing educational information on health/social services in Alameda County Informing clients of community services and contacting agencies on their behalf Assisting clients with navigating social and health systems, including application assistance for MediCal and other benefits; screening for eligibility; scheduling appointments Supporting professional staff with non-clinical interviews and data collection, entering information into systems Conducting outreach calls to newly enrolled participants to assess needs and make referrals Health education and community outreach on topics related to disease prevention and CHS programs, including participating in health fairs, conducting educational sessions, public speaking, and coalition participation Supporting policy change and community health initiatives by working with community leaders and organizations, educating staff, and advocating for healthier environments Providing problem-solving support for public benefits or insurance disruptions Representing staff in community meetings and translating educational materials as needed Interpreting during client interviews, maintaining records and data entry, and supporting case management services as assigned Performing other duties within the classification as assigned. MINIMUM QUALIFICATIONS Experience:
The equivalent of one (1) year of full-time experience in community outreach, including duties like intake, peer counseling, data collection, health education, screening, placement, and referral. Substitution:
An AA degree (60 semester or 90 quarter units) in health services, social sciences, health education, or related fields may substitute for one year of experience. HOW TO APPLY Deadline:
