Overview
Blended Case Management (BCM) is a service that assists young people with mental illnesses or emotional disorders by building a strong network of community resources and services. A Case Manager will link individuals with needed services in the community; advocate for children, youth, and young adults and their families; help negotiate complex systems such as special education and social services; assist families in maintaining basic living needs and skills such as employment, housing, food, medical care, and recreation; and provide on-site crisis intervention. The level, frequency, or type of case management service is determined by the intensity of each family's unique situation. The BCM Intake Specialist is responsible for providing training and support to BCM staff and coordinating intakes for new clients being referred to the Blended Case Management Program in Montgomery and Delaware County while maintaining a BCM caseload. Required Qualifications: A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science 2 or more years of experience in public or private human services with 1 year in direct client contact 1 or more years of experience working in a Blended Case Management position Preferred Qualifications: Strong leadership qualities, including but not limited to time management, work ethic, and passion for service Job Responsibilities: Maintain a BCM caseload of up to 20 participants performing all duties specific to a BCM position (this is variable based on the flow of referrals and ability to meet productivity requirements) Achieve at least 60 billable units per week Perform all required duties of a BCM Complete intakes for new BCM participants Offer BCM intake within 7 days of receipt of approved referral Consult with assigned BCM to share intake information Assist with staff training Support staff with case-specific needs Provide additional support for intensive cases Cover unstaffed BCM cases Participate in supervision with BCM leadership Be available a minimum of 3 evenings per week Perform other duties, when required or assigned Work Environment: Manage a varied 40-hour per week work schedule Monday-Friday between 8:30 AM - 8:30 PM, including at least 3 evenings per week Work in standard office environments, and community settings including client homes, schools, courts, doctor's offices, and hospitals.
