Reporting to the chief medical officer (CMO), the advanced practice nurse in collaboration with Ocean Health Initiatives, Inc. (OHI) staff physicians, will provide primary care services to adult, pediatric, and pregnant patients of OHI, following established policies, procedures, protocols, OSHA standards, and other safety and health regulations. Responsibilities
Coordinate and manage individual patient care in collaboration with the chief medical officer and staff physicians. Work cooperatively with all providers and staff to meet OHI's goals and meet productivity standards. Provide continuing care to assigned patients, recognizing deviations from normal, initiating treatment, and consulting with physicians as needed. Collect and record patient information, including medical history, reports, and examination results for pediatric, internal medicine, and OB/GYN patients. Adhere to best care practices aligned with guidelines from the American Board of Pediatrics and the American College of Obstetricians and Gynecologists. Order diagnostic tests, medications, and labs; analyze and interpret data for patient management. Educate and counsel patients, families, and community groups on health conditions. Function as part of a multidisciplinary healthcare team. Assist in developing, implementing, and monitoring clinic policies, procedures, protocols, and performance improvement programs. Ensure encounter productivity, quality measures, and family planning are met. Participate actively in departmental and medical staff meetings. Support colleagues with internal referrals when appropriate. Maintain accurate and timely documentation, including medical history, reports, and treatment plans, in accordance with OHI policies. Provide care regardless of patient's ability to pay, race, or creed, respecting patient rights, privacy, and dignity. Refer patients to specialists as needed, ensuring referrals comply with insurance restrictions. Develop and implement goals to ensure high-quality, cost-effective care. Complete all documentation within 24 hours of patient visits. Plan and enforce policies to ensure compliance with regulations and standards. Participate in performance improvement and safety programs. Willing to work evenings and weekends across all OHI sites. Establish and maintain effective working relationships with staff and providers. Work in various settings, including mobile units and traditional offices. Communicate effectively with diverse populations. Provide guidance and support to clinical support staff. Maintain professional skills through continuing education. Ensure patient satisfaction through compassionate communication and education. Adhere to ethical standards and confidentiality policies. Participate in safety programs and mandatory trainings. Collaborate with management to resolve challenges professionally and efficiently. Lead by example in departmental operations. Maintain transparency and accountability via project management tools like Monday.com. Adhere to corporate compliance policies. Maintain a monthly scorecard threshold of at least 80%. Perform other duties as assigned. Education/Experience/Licensure
Master's degree in Nursing from an accredited institution. 3-5 years of RN-level experience in healthcare. Active RN and advanced practice nurse licenses in New Jersey. At least 2 years of experience in Women's Health, including Obstetrics. Active CDS and DEA licenses for prescribing controlled substances. Certification as an advanced practice nurse by the New Jersey state board. BLS certification through the American Heart Association. Proficiency in Microsoft Office 365. Benefits
