We are looking for a new team member to join our intensive home visiting services team as a Home Visitor/Case Manager for our Healthy Families Program. As a home visitor/case manager, you will be responsible for building and sustaining a relationship with a caseload of diverse families who are pregnant or parents of newborns through the child's earliest years. This position requires travel throughout DuPage County. Dual language candidates are encouraged to apply. Responsibilities
Works with and provides services to young children and their families, by providing weekly home visits at the start of services. Outreaches families and builds and establishes trusting, long term (3-5 year) relationships with families. Manages a client caseload consistent with department productivity standards. Meets with clients in their homes and in the community throughout DuPage County. Also maintains communication with clients via phone, text, and video visits. Assesses family needs. Conducts assessments and screenings including parent interview about the familys story and screenings for developmental functioning, depression, substance use, and family violence. Connects families with appropriate resources. Utilizes reflective practices to support and strengthen parent-child interaction and overall family functioning. Helps families develop and implement practical approaches to real-life situations. Participates in weekly individual reflective supervision and monthly group reflective consultation. Completes and maintains required documentation within established time frames. Participates in emergency response activities as assigned. Maintains required training, licensure, and/or certification. Maintains confidentiality of privileged information and adheres to patient privacy laws. Demonstrates sensitivity and understanding of other ethnic groups and cultures. Adheres to department guidelines for attendance and punctuality. Performs community outreach activities. Requirements
A minimum of a high school diploma or equivalent; infant mental health credential preferred. Designated positions may be required to be bilingual, Spanish speaking, based on the population served. Must have dependable transportation, valid drivers license. Attendance at our on-site paid HR new hire orientation in Wheaton is required. Supplemental Information
Experience in working with or providing services to children and families. An ability to establish trusting relationships. Knowledge of infant and child development. Acceptance of individual differences. Experience and humility to work with the culturally diverse families. Willing to engage in building reflective capacity (e.g., capacity for introspection, communicating awareness of self in relation to others, recognizing value of supervision, etc.).
