Position Overview
We are seeking an experienced and compassionate Acute Response Sr. Manager to lead our Acute Response team, a key function supporting patients with urgent clinical needs. This role requires a licensed RN in the Boston area who thrives in a dynamic, team-oriented environment and is passionate about advancing care equity for vulnerable older adults.

Duties
Lead Acute Response Partners including hiring, onboarding, and performance oversight Oversee the day-to-day execution of urgent patient follow-up and intervention coordination Serve as the point of escalation for urgent or complex patient presentation via remote patient monitoring, phone, and in-home visits by Gather Medical Technician Partners (community EMTs and Paramedics) Ensure rapid and appropriate intervention via virtual, in-home, or in-clinic pathways Audit Acute Response Partner cases to ensure adherence to triage intake protocols Collaborate with PCPs and Care Team members to coordinate patient monitoring and close the loop on urgent care needs Lead standardized post-discharge workflow to schedule timely follow-up visits after hospitalizations Partner with PCPs and Care Team members to prevent readmissions and adverse outcomes Support continuous improvement of the Acute Response Team
Background
Registered Nurse (RN) licensed in the state of Massachusetts 5+ years of clinical experience including patient triage 3+ years of experience managing clinical teams and cross-functional coordination in a healthcare setting Strong interpersonal skills and the ability to lead with clarity, empathy, accountability, and urgency Comfort working in a tech-enabled, startup-like environment experiencing rapid change Prior experience in primary care-based triage and urgent care response workflows Strong analytical, problem-solving, and decision-making skills Experience in the Emergency Department, ICU, or as Charge Nurse a plus Experience working with older adults and community-based teams also a plus
