* Conduct history and physical examinations on new participants, as well as periodic reassessments. * Develop and implement medical treatment plans, delivering necessary services in various locations. * Evaluate applicants to the PACE Organization and engage in the Intake and Assessment process. * Discuss medical conditions and future care planning with participants and their caregivers. * Collaborate with nursing treatment programs and participate actively in interdisciplinary team discussions. * Educate participants and families about health maintenance and chronic conditions. * Supervise healthcare and treatment programs designed by the interdisciplinary team. * Perform physical examinations and order necessary laboratory and diagnostic tests. * Participate in provider on-call rotation as required.
Qualifications:
* Current and active plenary physician license to practice in Florida - REQUIRED * Board Certification in Internal Medicine or Family Practice - REQUIRED ; Geriatrics certification is preferred . * Experience or qualifications in geriatrics - Preferred . * CDS Certification and DEA Registration - REQUIRED * Previous supervisory experience - REQUIRED * Minimum of one year of experience working with frail or elderly populations. * Strong physical assessment and chronic disease management capabilities. * A passion for working in an innovative healthcare setting. * Able to adapt to changing priorities based on participant needs.
License and Certification Requirements:
* Active physician license in Florida * Board Certification in Internal Medicine or Family Practice * CDS Certification * DEA Registration
