Job Title: Home Health Aide I (HHA)

Schedule: Monday - Friday; Days, 8am-5pm

Join our compassionate team of caregivers dedicated to enhancing the quality of life for our patients by providing vital at-home support.

RESPONSIBILITIES:

  • Patient Care & Service: Promote and restore patients' health by executing tasks outlined in their care plan, collaborating closely with Community Health Nurse/Therapist, and providing assistance in personal care, light housekeeping, meal preparation, and running errands.
  • Communication: Effectively convey patient health statuses, treatment plans, and progress updates, while maintaining regular communication with office and supervisory staff.
  • Documentation: Ensure accurate and complete documentation of client care on the activity sheet.
  • Compliance: Adhere to departmental protocols, HIPAA regulations, and all local, state, and federal requirements.

REQUIRED QUALIFICATIONS:

  • Valid New York State Driver's License.
  • Home Health Aide certification from the New York State Education Department.

  • Strong critical thinking skills and ability to work independently.
  • Exceptional documentation and communication abilities.
  • Proficient in reading, writing, and speaking English, with the capability to follow written and verbal instructions.

EDUCATION:

LICENSES / CERTIFICATIONS:

  • Home Health Aide certification from the New York State Education Department.
  • Valid New York State Driver's License.

PHYSICAL REQUIREMENTS:

This position requires medium work, including exerting 20 to 50 pounds of force occasionally and/or 10 to 25 pounds of force frequently, with significant walking, standing, or squatting.

$18.65 - $19.75
