The Care Coordinator provides high quality, community-based Wraparound services to youth and families. Use your facilitation skills to engage with a multi-disciplinary team of professionals treating youth with behavioral health needs and their families to achieve stability and success in the community. Care Coordinators help families connect to community supports, navigate different systems (school, juvenile justice system, behavioral health systems etc.) and support through crisis interventions.

Intensive Care Coordination Program (ICC):
The Intensive Care Coordination (ICC) program at Behavioral Health Network is a voluntary outreach service that uses 'wraparound' principles to guide the work for youth with serious emotional or behavioral health needs. Wraparound is simply the coordination of multiple services via a tailored plan to address the needs of the youth.

What You'll Do:
As a
Care Coordinator I , you will facilitate care planning, connect families to essential resources, and provide crisis intervention to help youth with
Serious Emotional Disturbance (SED)
and behavioral health needs thrive in their communities.
Key Responsibilities: Provide
community-based Wraparound services
to youth and families. Collaborate with
multidisciplinary teams , including behavioral health professionals, schools, juvenile justice systems, and other support services. Assist families in
navigating behavioral health systems , connecting them to essential community resources. Facilitate
family and team meetings
to ensure individualized care coordination. Support youth and families through
crisis interventions
to promote safety and stability. Work within
Intensive Care Coordination (ICC) wraparound principles
to tailor treatment plans. What We Offer:

Bachelor's Degree
in a related field with
one year of experience
working with youth with
Serious Emotional Disturbance (SED)
OR
associate's degree
with
five years of experience
working with youth with SED. CANS-MA certification
(required). Strong knowledge of
mental illness and treatment techniques
for children and adolescents. Experience
facilitating meetings or groups
(preferred). Knowledge of
community resources and child/family-serving systems
(preferred).
