Description Conduct thorough evaluations of patients in inpatient or outpatient environments and formulate personalized treatment plans Implement diverse physical therapy interventions concentrating on mobility, strength, balance, and overall functionality Work closely with nurses, physicians, case managers, and fellow rehab professionals to enhance patient outcomes Guide patients and their families regarding therapeutic objectives, discharge planning, and at-home exercise regimens Maintain accurate records of evaluations, treatments, and progress, adhering to facility policies and regulatory requirements Engage in multidisciplinary team rounds and conferences to ensure seamless care coordination Education
Doctorate in Physical Therapy (DPT) or a comparable degree from a recognized program Certifications
Current state licensure as a
Physical Therapist CPR certification is preferred Specialized certifications (e.g., neurological, geriatric) are a plus, though not required Skills
Previous experience is highly preferred Capability to treat patients with complex medical and mobility challenges Excellent communication and teamwork abilities Proficiency in digital charting and clinical documentation systems Benefits
