Description Conduct evaluations for patients in both inpatient and outpatient settings while crafting tailored treatment strategies Deliver physical therapy interventions that emphasize mobility, strength, balance, and overall functionality Work alongside nurses, physicians, case managers, and other rehabilitation experts to improve patient outcomes Guide patients and their families through therapeutic objectives, discharge preparations, and at-home exercise plans Maintain meticulous documentation of evaluations, treatments, and patient progress in accordance with facility protocols and regulations Engage in team discussions and interdisciplinary meetings to ensure cohesive care for all patients Education
Doctorate in Physical Therapy (DPT) or equivalent from an accredited program Certifications
Active state licensure as a
Physical Therapist CPR certification is preferred Additional specialized certifications (neurological, geriatric) are advantageous but not mandatory Skills
Prior experience is highly preferred Able to manage patients with complex medical histories and mobility challenges Excellent communication and teamwork abilities Proficient in digital charting and clinical documentation systems Benefits
