Your Role and Responsibilities Conduct thorough assessments of patients in both inpatient and outpatient environments, crafting customized treatment strategies Implement comprehensive interventions aimed at improving mobility, strength, balance, and functional abilities Work collaboratively with a multidisciplinary team including nurses, physicians, case managers, and rehabilitation experts to enhance patient outcomes Educate patients and their families about therapeutic goals, discharge procedures, and home exercise regimens Meticulously document all evaluations, treatments, and progress in compliance with facility and regulatory standards Engage in care team rounds and participate in interdisciplinary conferences to ensure seamless patient care Qualifications

Doctorate in Physical Therapy (DPT) or equivalent credential from an accredited institution Licensing and Certifications

State licensure as a

Physical Therapist CPR certification preferred Any specialized certifications (neurological or geriatric) are advantageous but not mandatory Essential Skills

Prior experience is strongly preferred Expertise in managing patients confronting intricate medical conditions and mobility challenges Excellent communication abilities and a collaborative spirit Competence with digital charting and clinical documentation software Benefits
