Job Summary
The Social Worker MSW/LCSW is a clinician responsible for social work services for patients in various units of the Medical Center as well as serves in a consultative role to staff on the units in the medical center whenever needed for patient care. The Social Worker MSW/LCSW works as part of the interdisciplinary team caring/assisting patients and families to cope with plans and decisions that need to be made before, during and after hospitalization.
Responsibilities
- Provides individual and/or group counseling/therapy to patients and/or their families.
- Supervises and trains graduate level social work students and other students when indicated and approved by the Director of Social Work.
- Provides diagnostic evaluations and psycho-social assessments.
- Provides referrals and/or follow-up services.
- Collaborates with the clinical interdisciplinary team in facilitating continuing care plans.
- Provides crisis prevention when needed to patients in various units of the Medical Center.
- Provides counseling for patients, family and friends whenever indicated at time of diagnosis, regarding adjustment to illness, the emotional impact of diagnosis, and changes in lifestyle.
- Offers continuity by maintaining a supportive relationship to patient and family throughout the course of illness.
- When indicated, counsels and supports patients and other family in bereavement.
- Provides case management for patients, including resource findings, concrete services as indicated, and referrals to community agencies and assistance in obtaining financial aid.
- Provides documentation for social work activity in the patient\'s record on a regular and timely basis.
- Consults with members of the medical center team regarding the psycho-social needs of patients and participates in staff in-service.
- Leads or co-leads a support group for patients and/or family members when appropriate.
- Participates in community organizations relevant to the population where possible.
- Performs psychosocial assessments of high-risk clinic patients as time allows.
- Assists in crisis situations and with concrete services as needed; refers patients wishing to place their baby for adoption to the appropriate agency.
- Assesses discharge needs of all patients admitted to their department assignment.
- Performs other duties as assigned.
Job Requirements
Educational and Experience Requirements
Master’s degree in social work and 0-2 years of experience working with patients and families who are in crisis; or an equivalent combination of education and experience.
Preferred Education and Experience
1-2 years of experience with patients with major trauma, infectious disease, and/or emotional upset.
Certifications, Licenses, Registrations Required
Current Licensed Clinical Social Worker in the state of Massachusetts
Certifications, Licenses, Registrations Preferred
LICSW preferred
Knowledge, Skills & Abilities (KSAs)
Ability to work with patients and families who are in crisis.
