The Social Worker is a clinician responsible for providing social work services to the patient population at St. Elizabeth's Medical Center.
This position reports to the Director of Social Work and operates with a high degree of autonomy, utilizing specialized expertise in psychiatric services.
The Social Worker collaborates with all staff on medical units as needed for patient care, quality management, committees, and focus groups.
Serves in a consultative role to staff, assisting with patient care whenever required.
III. Authority: Provides social work services on designated units, helping patients and families cope with plans and decisions before, during, and after hospitalization.
Supervises and trains graduate-level social work students and other students as approved by the Social Work Supervisor and upon meeting supervision requirements.
IV. A. Responsibilities/Essential Functions:
- Provides excellent customer service to internal and external clients, customers, and patients, aligned with the Service Excellence Standards.
- Provides individual and group counseling/therapy to patients and families.
- Supports patient advocacy efforts.
- Conducts diagnostic evaluations and psycho-social assessments.
- Provides referrals and follow-up services.
- Collaborates with interdisciplinary clinical teams.
- Facilitates continuing care planning.
- Offers crisis prevention services.
- Supervises other social workers and graduate students.
- Offers counseling to patients, families, and friends regarding diagnosis, adjustment to illness, emotional impact, and lifestyle changes.
- Maintains supportive relationships with patients and families throughout illness.
- Provides bereavement counseling when indicated.
- Manages cases by identifying resources, providing concrete services, and referring to community agencies, including assistance with financial aid.
- Documents social work activities promptly and accurately in patient records.
- Consults with medical team members regarding psycho-social patient needs and participates in staff in-service training.
- Leads or co-leads support groups for patients and families when appropriate.
- Engages with community organizations relevant to the patient population.
- Performs psycho-social assessments of high-risk clinic patients, prioritizing problems within limited time frames.
- Provides support to high-risk patients, including those with mental illness, developmental disabilities, histories of abuse or neglect, domestic violence, or substance abuse, especially during pregnancy.
- Provides crisis intervention and assists with concrete services as needed.
- Supports patients wishing to place a baby for adoption by referring them to appropriate agencies.
Minimum Education: A master's degree from an accredited school of social work is required.
Preferred Qualifications: LCSW or LICSW licensure is preferred.
