Responsibilities Provide emotional support and counseling to clients in need.
Develop and implement individualized care plans for clients.
Conduct assessments to identify client needs and resources.
Advocate for client needs within the healthcare system.
Collaborate with healthcare professionals to ensure comprehensive client care.
Participate in team meetings to discuss client progress and strategies.
Engage in continuous professional development and training.
Monitor and evaluate client progress towards goals.
Ensure compliance with all relevant regulations and ethical standards.
Maintain accurate and up-to-date client records and documentation.
Stay informed about community resources and refer clients as needed.
Utilize data and metrics to measure success and improve services.
Experience/Qualifications Valid state license to practice as a Social Worker in North Carolina.
Master's degree in Social Work or related field.
Strong communication skills for client advocacy and collaboration.
Commitment to continuous learning and professional development.
Minimum of 2 years experience in social work or healthcare setting.
Proven ability to develop and implement effective care plans.
Demonstrated ability to work independently and as part of a team.
Ability to handle high-pressure situations and meet performance targets.
Proficiency in using electronic health records and client management software.
Knowledge of community resources and ability to make appropriate referrals.
