Responsibilities
- Administrative Functions: Assist the Director of Nursing Services in directing the day to day functions of the nursing activities in accordance with current rules, regulations, and guidelines that govern the long term care facility. Develop, maintain, and update written policies and procedures for the nursing service department. Ensure reference material (e.g., PDRs, regulations, professional standards of practice) is current. Recommend materials to support resident care. Ensure the Nursing Service Procedures Manual is current and followed. Make reports/recommendations to the Director as necessary. Review department policies, procedures, and job descriptions periodically and recommend revisions. Coordinate nursing services with other resident services to maintain continuity of care. Ensure compliance with job descriptions. Participate in quality assurance and facility surveys. Review and update resident discharge plans as required and assist in planning the nursing services portion of discharge plans. Plan department services and activities with staff and support personnel. Admit, transfer, and discharge residents as required. Complete accident/incident reports as necessary. Interpret policies to personnel, residents, visitors, and government agencies. Perform administrative duties (forms, reports, evaluations, studies, charting, etc.). Protect resident information and ensure security of access to information systems.
- Committee Functions: Attend facility committee meetings (e.g., Infection Control, Policy Advisory, Pharmaceutical, Quality Assessment). Prepare reports on nursing service programs and activities. Provide information to the Quality Assurance and Assessment Committee. Assist in developing agendas for Care Plan Committee meetings. Implement committee recommendations as directed.
- Personnel Functions: Ensure adequate staffing of licensed practical and/or registered nurses and certified nursing assistants. Develop and assign work tasks; delegate authority to Charge Nurses. Ensure CNA graduates or trainees are supervised. Perform daily rounds; report findings. Monitor absenteeism and staffing levels. Ensure compliance with dress codes and policies. Address complaints and grievances; report as necessary. Foster cross-department collaboration and a positive unit atmosphere. Lead shift meetings, evaluate workforce, and participate in performance evaluations and staffing decisions. Administer fair disciplinary actions. Report occupational exposures and incidents per policy. Ensure secure access to information systems and protect resident privacy.
- Nursing Care Functions: Inform staff of admissions and room assignments; ensure rooms are ready. Greet and accompany new residents, coordinate with physicians, and observe residents daily. Ensure direct nursing care by licensed nurses or qualified aides. Review notes for accuracy and alignment with resident wishes. Start IVs and collect specimens as ordered. Admit, transfer, and discharge residents; provide status reports. Authorize restraints per policy. Oversee medication administration and charting; ensure accurate orders and adherence to stop orders. Inspect treatment areas and ensure safety. Ensure call-for-help responsiveness and regular family updates. Arrange transportation for discharge; notify families of deaths.
- Staff Development: Plan, conduct, and schedule in-service training. Support ongoing education (OSHA, HIPAA, Infection Control, Safety, etc.). Plan clinical supervision for nurse aide trainees. Participate in continuing education to maintain licensure.
- Safety and Sanitation: Develop safety standards for the nursing service; ensure training for exposure to blood/body fluids. Maintain PPE stock and enforce proper use. Enforce hand hygiene, conduct fire and disaster drills, and support the infection control program. Ensure proper reporting of hazardous conditions and use of safety equipment. Keep areas clean and sanitary. Ensure staff are trained for infection control procedures when isolation is required. Document MSDSs for hazardous chemicals. Ensure appropriate equipment operation and waste avoidance.
- Equipment and Supply Functions: Advise on equipment and supply needs; maintain stock levels. Support preventive maintenance with Facilities. Ensure only trained personnel operate equipment. Promote efficient use of supplies and maintain MSDS records.
- Care Plan and Assessment Functions: Develop preliminary and comprehensive assessments; involve residents in care plan development. Review and update resident goals and plans. Ensure staff follow the care plan; coordinate with the MDS process, RAPs, and Triggers. Verify care aligns with resident wishes and treatments.
- Budget and Planning Functions: Monitor economic conditions and adjust nursing services to maintain quality care. Identify equipment needs and report fraud concerns when appropriate.
- Supervisory Requirements: Assist with overall supervision and management of nursing staff. The RN is delegated administrative authority and accountability for assigned duties.
Qualifications
- Education and/or Experience: Nursing degree from an accredited institution. Training in rehabilitative and restorative nursing. 2+ years experience, preferably in a long-term care facility.
- Language Skills: Proficient in reading, writing, speaking, and understanding English. Able to read technical procedures and policy manuals; able to present information and respond to questions.
- Mathematical/Reasoning: Ability to apply fractions, percentages, ratios, and proportions; independent decision-making; problem-solving; tactful interaction with residents, families, staff, and agencies; strong leadership and supervisory ability; ability to plan, organize, and implement programs and policies for quality care; knowledge of geriatrics and regulatory requirements.
- Certifications/Licenses: Current, active RN license in state; current CPR certification. Knowledge/experience with PCC preferred. Maintain continuing education/licensing and stay in good standing with the State Board of Nursing.
- Physical Demands: Ability to move intermittently; communicate effectively in English; withstand mental and emotional stress; vision and hearing adequate for job; ability to lift/move at least 25 pounds over distances as specified; ability to assist in evacuations if needed.
- Work Environment: Works across multiple facility areas; may require evenings, weekends, holidays; potential exposure to infectious materials; compliance with safety and infection control protocols. May be exposed to hazardous chemicals and physical hazards. May require attendance at trainings and continuing education.
