Physical Therapist (PT) – Long Term Care/SNF - Travel Position
Location: Pocatello, ID Job Type: Travel Contract Duration: 13 weeks Shift: 5x8s Pay: $2,305/week
Job Description:
We are seeking a compassionate and skilled Physical Therapist (PT) to join a skilled nursing and long-term care facility in Pocatello, ID, on a travel contract. The PT will evaluate residents, create individualized treatment plans, and provide therapy services designed to improve mobility, enhance functional independence, and support quality of life. The ideal candidate is detail-oriented, patient-focused, and experienced in geriatric rehabilitation.
Key Responsibilities:
Conduct comprehensive patient evaluations to assess physical function, limitations, and therapy needs.
Develop and implement individualized treatment plans in accordance with physician directives and facility protocols.
Provide therapeutic interventions including strengthening, balance, gait training, and functional mobility exercises.
Educate residents, families, and caregivers on therapy plans, safe mobility techniques, and use of assistive equipment.
Document evaluations, progress notes, and outcomes in compliance with facility and regulatory requirements.
Collaborate with interdisciplinary teams to ensure coordinated and effective care delivery.
Promote patient safety, independence, and overall well-being through evidence-based therapy practices.
Qualifications:
Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.
Current and valid Physical Therapist license in Idaho (or compact license).
Minimum of 1 year of PT experience, preferably in skilled nursing or long-term care.
Current Basic Life Support (BLS) certification.
COVID vaccine and recent TB test.
Strong communication, assessment, and organizational skills.
Compassionate and patient-centered approach, especially with geriatric populations.
