Description Conduct thorough evaluations for patients in both inpatient and outpatient settings, crafting personalized treatment plans to guide their recovery Implement physical therapy interventions that enhance mobility, strength, balance, and overall function Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimal outcomes Guide and educate patients and their families on therapeutic objectives, discharge planning, and home exercise routines Maintain accurate documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards Engage in care team rounds and multi-disciplinary conferences to ensure seamless coordination of care Education

Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program Certifications

State licensure as a

Physical Therapist CPR certification is preferred Specialty certifications (e.g., neurological, geriatric) are a plus but not mandatory Skills

Prior experience is highly regarded Able to care for patients with intricate medical and mobility challenges Excellent communication and teamwork capabilities Proficiency in using digital charting and clinical documentation software Benefits
