Position Overview:

LHH is currently seeking experienced Housing Case Managers for our client in LA. At this organization, you will play a critical role in guiding clients through complex housing issues and situations by assessing their needs, developing tailored action plans, and coordinating with various service providers. You will be responsible for ensuring that clients receive the appropriate resources and support to achieve their goals and improve their quality of life.

Hiring method: Contract to Hire

Location: South Los Angeles / Long Beach (multiple resident location options)

Pay rate: $24-25/hour

Hours: Monday - Friday, 8AM-5PM

Responsibilities include:

Client Assessment: Evaluate client needs and strengths to create care plans.

Care Planning: Develop and update personalized care plans.

Resource Coordination: Link clients to community services and support.

Case Management: Track progress and adjust plans as needed.

Advocacy: Ensure clients receive necessary services and resolve barriers.

Documentation: Keep accurate and confidential records.

Collaboration: Partner with professionals for coordinated care.

Compliance: Follow all legal and organizational standards.

Qualifications include:

Bachelor degree

6 months or more of experience working directly with the homeless

Pay Details:

$24.00 to $25.00 per hour
