Field-based hospice care (patient homes, assisted living, skilled nursing facilities) Start Date:
October 13, 2025 End Date:
January 10, 2026 Duration:
13-week nursing contract Shift:
8-hour day shifts (8:00 AM - 5:00 PM), Monday-Friday Hours per Week:
40 guaranteed Estimated Weekly Salary:
$1630 - $1750 Patient Visits:
Average 5-6 visits per day (goal of 25 per week) Autonomy:
Manage your own schedule Mileage reimbursement
at current IRS rate Registered Nurse Qualifications
Active Registered Nurse (RN) license in North Carolina BLS certification (Basic Life Support) Valid driver's license and vehicle insurance Minimum 1 year of hospice nursing experience in home health or home visit setting Experience with Homecare Homebase EMR (Electronic Medical Records) Strong communication, assessment, and patient education skills Clinical skills in catheter care, feeding support (including g-tubes), CAD pumps, medication management, and psychosocial support Hospice Nurse Responsibilities
Deliver compassionate end-of-life nursing care to hospice patients in homes, assisted living, and skilled nursing facilities Conduct comprehensive patient assessments, develop and implement individualized care plans, and educate patients and families on the hospice process Participate in biweekly interdisciplinary team (IDG) meetings and complete all required clinical documentation Provide bereavement and psychosocial support to patients and families Collaborate with a multidisciplinary healthcare team including nurse educators, clinical supervisors, social workers, case managers, nurse practitioners, chaplains, and home health aides Utilize provided technology (tablet and laptop) for charting and communication Ensure compliance with all regulatory and organizational standards for hospice care
