Overview

Physician Family Medicine - ACCESS Warren at Access Community Health Network. Provide the full scope of Family Medicine services, including diagnosis, treatment, coordination of care, preventive care, and electronic health record (EHR) maintenance, following Access Community Health Network guidelines and quality programs. Responsibilities

General Clinical: Obtain health history and perform physical examinations; diagnose, treat, counsel, and coordinate care for patients; update problem lists and longitudinal plans; meet documentation standards and complete progress notes within 24 hours; fulfill on-call responsibilities and respond to calls within 20 minutes; prescribe medications per Illinois statutes and guidelines; conduct care in accordance with ACCESS quality standards; meet Meaningful Use standards to improve quality and coordination; manage EHR In-Basket and respond to messages within 72 hours. Patient Support and Education: Provide culturally competent care; involve families as part of care teams; ensure patient and family needs are met; educate patients and families, and promote self-management; use appropriate health education materials and tailor explanations to patient understanding; coordinate with care teams and case management for delivery of services; transfer patients needing higher acuity appropriately; meet intensity of service criteria. PCMH Requirements: Coordinate with center staff to meet patient needs; participate in health center huddles and productivity standards; ensure timely and appropriate patient visits with involvement of medical assistants and front desk staff; ensure care teams carry out effective treatment and education in line with policies. Provider Development: Participate in staff orientation and training; ensure internal certification requirements for patient education and medication administration; participate in proficiency testing; may precept learners; adhere to HRSA and FTCA regulations; perform other duties as assigned. Requirements/Preferences: Doctor of Osteopathy (D.O.) or Doctor of Medicine (M.D.) required; completed residency in primary care with Board Certification; minimum two years in outpatient primary care preferred; current Illinois licensure, DEA, and CME maintenance; knowledge of quality, safety, and infection control rules; EPIC/EMR competency preferred. Working Conditions/Equipment: Health center environment; ability to concentrate amid interruptions; ability to stand, sit, and walk for extended periods; operate a computer and standard office equipment. Compensation and Benefits
