Overview
Clinical Nurse (RN) -Transfer Center - 0.9 FTE, 12 HR, Night Shift. Transfer Center Nurses provide 24-hour coverage in the Transfer Center to facilitate smooth transfer or transport and transition for all emergent, urgent, and elective patients into the Stanford University Medical Center from referring hospitals, clinics, and physician's offices ensuring they are provided the appropriate level of care bed, financial screening, and timely, appropriate mode of transport. Transfer Center Nurses evaluate, process, track, document, and coordinate patient transfers and patient transports into and out of the Stanford Medical Center. Transfer Center Nurses efficiently engage appropriate departments in the transfer process and are responsible for oversight of the entire transfer episode, from start to finish. Location: Stanford Health Care, 500 Pasteur Dr, Palo Alto, CA. Night - 12 Hour (United States of America). This is a Stanford Health Care job. Note: Advanced Cardiovascular Life Support (ACLS) certification may be required by the cost center in which the position resides. Base Pay Scale: Generally starting at $92.64 - $106.87 per hour. Salary of the finalist will be set based on factors including internal equity, experience, education, specialty and training. Responsibilities
Contacts Case Manager at insurance or review organization to establish case management. Coordinates emergency admission clinical issues as needed. Coordinates with staff of other hospital departments to ensure patient needs are met from admission through discharge. Documents findings and recommendations for accepted transfers or transports. Performs nursing assessment to determine appropriate level of care and need for specialized equipment; provides assessment information to the accepting unit and coordinates arrival time; assesses, implements, and evaluates interventions based on patient status. Provides expert clinical decision-making to determine patient acuity, need for transfer, and appropriate means and level of transfer or transport, and communicates with hospital physicians and nurses. Provides nursing care to patients with acute or complex conditions or in emergencies per Transfer Center protocols. Screens patients for discharge planning needs and coordinates with referring institutions and community resources. Acts as an expert resource and consultant to other nurses, physicians, departments, and families. Confers with Admitting Physician and Social Workers or Case Managers regarding medical necessity, psychosocial needs. Qualifications
Graduation from an accredited school of nursing. Three (3) years of progressively responsible and directly related work experience. Knowledge, Skills And Abilities
Knowledge of EMTALA and related regulations. Knowledge of requirements and regulations of third-party payers (e.g., Medi-Cal, Medicare, CCS) and private insurance. Licenses and Certifications
Nursing – Registered Nurse (RN) – State Licensure And/Or Compact State Licensure. BLS – Basic Life Support. Experience and Culture
