Overview Bill Wilson Center provides services to children, youth, young adults, and families in Santa Clara County. Programs focus on housing, mental health care, supportive services, and advocacy. Job Summary Under the supervision of the Program Manager, the Lead Case Manager is responsible for data quality of clients\' files (paper and electronic in AWARDS and HMIS), running reports in AWARDS and HMIS, supporting the team, and providing general case management around education, housing, parenting, supportive counseling, and connections to needed resources and services to clients in preparation for independent living.
Essential Duties / Responsibilities
Conduct intake and assessment services.
Provide case management services to young parents in program.
Assist young adults in skills-building including independent living skills and parenting skills.
Assist young adults with developing educational and vocational plans.
Assist young adults with locating appropriate child care resources and making arrangements for the proper care of their children.
Provide referral assistance to legal services (BALA), health services, drug and alcohol services and mental health services.
Assist young adults in developing and implementing an Individualized Case Plan to meet long-term housing and independent living needs.
Utilize community resources to meet needs of young adults in the program.
Monitor clients’ housing conditions (residences) to ensure safety and/or cleanliness issues are addressed in a timely manner; report maintenance issues as needed.
Work with outside agencies to ensure quality of care for young adults and their children.
Transport clients to appointments, meetings, etc. as needed.
Supervise a Resident Mentor and volunteers assigned to the program.
Approve visitors, visitor passes, and overnight passes; enforce house rules.
Monitor safety concerns and address issues with residents as needed.
Participate in afterhours and weekend On-call rotation and crisis intervention.
Facilitate weekly psycho/educational parenting groups and house meetings.
Participate in agency and program meetings and complete required trainings.
Participate in Quality Assurance/Quality Improvement activities.
Maintain client files and data to agency standards (paper and electronic in AWARDS); complete reports as appropriate.
Follow agency protocols regarding accounting and financial requests, ensuring expenses are approved with proper documentation.
Communicate effectively with coworkers and management; perform under pressure and maintain a safe work environment by reporting safety issues promptly.
Regular attendance is required.
Additional duties as assigned.
Education and Experience
Bachelor's degree (B.A., B.S.) from a four-year college or university; or an equivalent combination of education and experience. Experience working with culturally diverse youth and parenting youth is required.
Required Skills / Abilities
Language Skills: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals; ability to write routine reports and correspondence; effective communication skills. Some positions require bilingual (English/Spanish or English/Vietnamese).
Certificates, Licenses, Registrations: Valid driver's license with access to a vehicle and personal auto insurance; current CPR/First Aid certification (or within 30 days of hire); fingerprint/background checks; TB test; COVID-19 vaccination with booster if eligible or eligible to receive booster.
Mathematical Skills: Basic mathematical concepts for reporting.
Reasoning Ability: Problem solving and interpreting varying instructions.
Computer Skills: Proficiency with Internet software, spreadsheets, word processing; internet research is a plus.
Physical Demands
Regularly sit, stand, use hands, reach, talk, and hear. May need to lift up to 20 pounds. Visual requirements include close and color vision.
Work Environment
Typical office/work environment with reasonable accommodations as needed. Noise level is usually moderate.
Salary $29.00 - $32.00 per hour.
