Job Summary

The Chronic Registered Nurse is accountable for delivering and coordinating clinically competent care for patients with End Stage Kidney Disease (ESKD). This role includes patient assessment, care planning, implementation, and evaluation, along with leadership and patient/family education responsibilities. The RN plays a key role in improving patient outcomes and supporting facility operations.

Key Responsibilities

  • Deliver all essential nursing functions per company policy and regulatory standards
  • Complete systematic assessments before, during, and after dialysis treatment
  • Update care plans and coordinate with supervisors and physicians as needed
  • Educate patients and families on treatment plans and options
  • Administer medications per prescription and track effectiveness
  • Act as an emergency response resource when required
  • Guide and support onboarding of new staff
  • Oversee lab collections and ensure proper processing and shipping
  • Collaborate with interdisciplinary teams to set care goals and revise plans of care
  • Promote transplant awareness and treatment modality education
  • Perform additional duties as assigned by Supervisor

Required Experience

  • At least 1 year of Nephrology Nursing experience within the past 2 years
  • Chronic hemodialysis experience required
  • Minimum 9 months general RN experience and 3 months dialysis RN experience

Nice-to-Have Experience

  • Experience in leadership or charge roles
  • Exposure to patient education for kidney transplant candidates

  • Hemodialysis delivery and monitoring
  • Medication administration
  • Patient evaluation and care planning

Preferred Skills

  • Emergency readiness and response
  • Staff mentoring and education
  • Interdisciplinary communication
  • Documentation using EMR systems
  • Knowledge of transplant and dialysis education

Additional Skills

  • CPR/BLS Certification
  • Ability to lift equipment and assist with patient movement (up to 200 lbs. with aid)
  • Frequent standing, bending, and walking
  • Willingness to travel to training or facility sites
  • Compliance with Ishihara Color Blindness test (accommodation available)
