• Shift: Days, Monday – Friday, 8:00am – 5:00pm
• Weekly Hours: 40
• Assignment Duration: 13 weeks (Start: 09/29/2025 – End: 12/28/2025)
• Patient Ratio: 1:1 (Up to 6 home visits per day; 30 productivity points standard)
• Case Load: Geriatric patients
• Estimated Weekly Salary Range: $2,320 - $2,380

Qualifications

• Active California Registered Nurse (RN) license required
• At least 1 year of recent skilled Home Health RN experience (within the last 3 years)
• Proficient with OASIS documentation for start of care and ongoing visits
• BLS certification (American Heart Association preferred)
• Reliable transportation for travel within a 50+ mile radius (client visits grouped to minimize drive time)

Responsibilities

• Deliver skilled nursing care to geriatric patients in their homes throughout the Burbank, CA area
• Conduct comprehensive patient assessments and develop individualized care plans
• Complete accurate OASIS documentation for all patient visits
• Collaborate with interdisciplinary healthcare teams to optimize patient outcomes
• Educate patients and families on disease management, medication administration, and home safety
• Maintain timely and thorough documentation of all patient interactions and care provided
