Responsibilities

• Deliver skilled nursing care to geriatric patients in their homes
• Conduct comprehensive patient assessments and develop individualized care plans
• Complete OASIS documentation for start of care and follow-up visits
• Collaborate with interdisciplinary healthcare team members to ensure high-quality patient care
• Educate patients and families on disease management, medication administration, and home safety
• Travel between patient homes within the assigned Orange County territory

Qualifications

• Active California Registered Nurse (RN) license required
• Minimum 1 year of recent skilled Home Health RN experience (within the last 3 years)
• OASIS documentation experience required for start of care
• Current Basic Life Support (BLS) certification
• Reliable transportation for travel nursing assignments (occasional travel up to 50+ mile radius; system minimizes drive time)

• Estimated Weekly Pay: $2400 - $2460
• Patient Ratio: 1:1 (up to 6 patient visits per day; 30 productivity points standard)
• Case Load: Geriatric patients
