Home Health RN Job Details

• Location: Eureka, CA (95501) – Northern California home health nursing job in a vibrant coastal city.
• Healthcare Setting: Home Health
• Assignment Duration: 13 weeks (Start: 9/29/2025 – End: 12/28/2025)
• Shift: Days, Monday–Friday, 8:00am–5:00pm (8-hour shifts)
• Hours per Week: 40
• Estimated Weekly Salary: $2320 - $2380
• Patient Caseload: Geriatric patients; up to 6 home visits per day (30 productivity points standard)
• Travel Radius: Occasional maximum radius of 50+ miles; efficient scheduling to minimize drive time

Registered Nurse Qualifications

• Active California Registered Nurse (RN) license
• At least 1 year of recent skilled home health RN experience (within the last 3 years)
• Proficient with OASIS documentation for start of care and ongoing visits
• Current Basic Life Support (BLS) certification
• Reliable transportation required for home visits

Home Health RN Responsibilities

• Deliver skilled nursing care to geriatric patients in their homes throughout Eureka, CA
• Perform comprehensive patient assessments and develop individualized care plans
• Complete accurate OASIS documentation for all patient visits
• Collaborate with interdisciplinary healthcare team members for optimal patient outcomes
• Educate patients and families on health management, disease prevention, and self-care techniques
• Maintain timely and thorough clinical documentation in compliance with home health standards
