Home Health Registered Nurse (RN) – Travel Nursing Job in Irvine, California. Seeking a skilled Registered Nurse (RN) for a home health travel assignment in Irvine, CA (zip 92612). This opportunity is for compassionate RNs to provide quality homecare to geriatric patients while enjoying the lifestyle and amenities of Southern California.

Responsibilities

• Deliver skilled nursing care to geriatric patients in their homes throughout Irvine, CA
• Perform comprehensive patient assessments and develop individualized care plans
• Complete accurate OASIS documentation for all patient visits
• Collaborate with interdisciplinary healthcare team members to coordinate patient care
• Educate patients and families on disease management, medication administration, and home care procedures
• Maintain timely and accurate clinical documentation in compliance with home health regulations

Qualifications

• Active California Registered Nurse (RN) license
• At least 1 year of recent skilled home health nursing experience (within the last 3 years)
• Proficiency with OASIS documentation for start of care and ongoing visits
• BLS (Basic Life Support) certification required
• Reliable transportation for travel nursing assignments

• Patient Population: Geriatric patients; up to 6 home visits per day (30 productivity points standard)
• Travel Radius: Occasional maximum of 50+ miles; patient visits are grouped to minimize drive time
