The Integrated Care Center Clinic is a federally qualified health center that provides primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We provide care to those most in need including those who are suffering from homelessness, mental illness, substance use, and chronic medical conditions. We provide team-based care in an integrated care setting. We are looking for a mission driven, service leader who works well in a team, has experience with managing and supporting staff, champions quality improvement, leads change, and is excited to innovate. The Enhanced Case Management (ECM) program under the Integrated Care Center will provide intensive, client-centered case management to support clients in achieving stability, health, and self-sufficiency.

Responsibilities

Client Care and Care Coordination : The Care Manager will organize patient care activities, share information with the multi-disciplinary care team, and implement activities identified in the Managed Care Plan.

Identify eligible people from within State Prisons, County Jails, hospitals, and other locations for enrollment into the MCP.

Engage clients using multiple strategies (in-person, mail, email, texts, telephone, outreach in the community).

Maintain regular contact with all providers on the member’s care team, including Justice Involved oversight.

Ensure continuous, integrated care among all service providers and coordinate with primary care, physical and developmental health, mental health, SUD treatment, and housing and other supports as needed.

Support client engagement in treatment, including medication review/reconciliation, appointment scheduling, reminders, transportation coordination, and addressing barriers to engagement.

Engage with a multi-disciplinary team to identify gaps and coordinate primary, behavioral, developmental, oral health, and community supports.

Assist with intake by completing assessments as required by the MCP.

Monitor progress toward care plan goals and provide input on treatment plans.

Assess client needs and complete all Releases of Information.

Connect clients to benefits, healthcare services, employment, housing, community resources, and outpatient SUD services.

Coordinate communication with external services, including scheduling appointments, probation, Family and Children Services, and court-related matters.

Collaborate with clients and families to support reintegration into the community.

Collaborate with outside agencies to obtain needed community resources (housing, sober living, recovery, medical facilities, employment/education, DPSS, DMV, etc.).

Clinical Documentation And Administrative Responsibilities

Write and complete all progress notes within 24 hours of service delivery.

Write progress letters and court reports for clients.

Complete all assigned peer reviews (chart audits) in a timely manner.

Actively participate in group supervision, agency, and team meetings.

Participate in training opportunities and complete assigned training in a timely manner.

Training, Supervision And Quality Improvement Responsibilities

Attend and actively participate in assigned case conferences to advocate collaborative treatment needs.

Participate in agency and team meetings and training opportunities.

Communicate collaboratively with all members of the behavioral health team (medical, mental health, psychiatry, substance use disorder, and other staff).

Complete all assigned training and Relias course requirements in a timely manner.

Field-Based Responsibilities This is a field-based position requiring frequent travel. A major component of the Lead Care Manager role involves meeting ECM members where they are, including in their homes and other locations. The role may include access to an office space for case management activities, clinical supervision, and attendance at required trainings and meetings.

Qualifications Education, Certification, or Licensure

Case Manager I: High school diploma or equivalent required.

Case Manager II: Bachelor’s degree in a human services-related field (social work, public health, psychology).

Minimum of two years’ experience in the human service field; demonstrated expertise in substance use treatment, relapse prevention, and recovery preferred.

Current First Aid and CPR certification.

Valid California driver’s license and access to registered and insured transportation.

Experience working with clients experiencing acute withdrawal from substances; experience delivering trauma-informed services preferred.

Background Check And Other Requirements

Qualified candidates with arrest and conviction records will be considered for employment.

Must be able to complete a background check and livescan.
