Job Title: Physical Therapist Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: - Recent Home Health experience - Reliable transportation - Current driver's license - Agency-required liability insurance Description: Home visits are required for this position. Periodic office meetings will be held. The average number of patients per day is 5-6. Each type of visit and activity has a different point value. Contractors will be assigned between 5-10 patients per day. This equates to roughly 6-8 points per day. There are no guaranteed hours. In cases of low patient census, please discuss options with the manager. Types of cases include Start of Care and Follow Ups. Weekend requirements vary by location and profession. Traveling up to 75 miles from the home clinic is required. Mileage will be reimbursed at the IRS standard rate. Contractor will be compensated from patient home to patient home. Commuting to and from the first and last patient is not billable. All candidates must understand that they will be responsible for driving to patients' homes daily, as well as to the clinic for weekly meetings. Candidates must have a vehicle, driver's license, and auto insurance. Longer drives between patients may occur. Candidates must accept every patient on their schedule regardless of distance from the previous appointment. Documentation must be completed within 24 hours of the appointment. Delayed or incomplete documentation may impact billing and patient care. There are no guaranteed hours with this contract. The candidate must hit weekly productivity points of 30, equating to 40 hours per week. All overtime must be pre-approved. If the candidate is not receiving enough patients each week to meet their financial needs, they will have the opportunity to float to another nearby clinic to pick up patients.
