Travel Physical Therapist (PT) - $2,440 per week in Fort Wayne, IN

Job Title: Physical Therapist Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience required License: Must have license in hand Certifications: CPR, BLS required Must-Have: Recent Home Health experience Description: Home visits and periodic office meetings are required. Reliable transportation is necessary. Current driver's license and agency-required liability insurance are mandatory. Average number of patients per day is 5-6. Contractor will be assigned between 5-10 patients per day. This equates to roughly 6-8 points per day. Each type of visit and activity (documentation, travel, etc.) holds a different point value. Guaranteed hours are not provided. If the contractor is unable to meet financial needs due to low patient census, they can discuss with the account manager. Types of cases include Start of Care and Follow Up visits. Weekend requirements vary per location and profession and should be discussed during the interview. Must be willing to travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. Contractor is compensated for travel between patient homes. Commute to and from the first and last patient is not billable. Local candidates are accepted at the same rate. No pending licenses are accepted; candidates must have a license in hand. This position requires acceptance of every patient on the schedule, regardless of distance. Documentation must be completed within 24 hours of the appointment. There are no guaranteed hours with this contract. Clinicians must achieve weekly productivity points of 30. Documentation delays or incomplete records impacting billing may result in cancellation of patients. The contractor is expected to have a vehicle, driver's license, and auto insurance. Mileage radius for this job is 50-75 miles, which may result in longer drives between patients. Travel experience is beneficial for this role. Experience with HomeCare HomeBase EMR is preferred. No corporate employees are eligible for this role as a traveler. Clinicians must be open to traveling up to 75 miles away from the assigned care center. Home Health checklist requirements must be satisfied. On-call rotations may be expected; however, specifics should be discussed with management.
