Job Title: Physical Therapist Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 year Adult Home Health Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience Description: Home visits are required for this position. Periodic office meetings will be held. Candidates must have reliable transportation and a current driver's license. Agency-required liability insurance is mandatory. The average number of patients seen per day is 5-6. Contracts will involve a set point system based on visits and activities (documentation and travel). Each contractor will be assigned between 5-10 patients per day, equating to roughly 6-8 points. If contractors are unable to meet their financial needs due to low patient census, they should discuss floating to nearby clinics for additional hours. Types of cases include Start of Care and Follow Ups. Weekend requirements vary by location and must be discussed during the interview. On-call responsibilities may also vary. Candidates must be willing to travel up to 75 miles from their designated care center. Mileage will be reimbursed at the IRS standard rate. Local candidates are accepted under the same conditions. Pending licenses are generally not accepted unless arrangements for a short delay are confirmed. Candidates should be aware this is a home health assignment and they will be responsible for driving to patient homes and attending clinic meetings. Comfort with potential longer drives between patients (20 minutes or more) is necessary. Candidates must accept every patient on their schedule, regardless of distance from previous appointments. All documentation must be completed within 24 hours post-appointment. Delayed or incomplete documentation impacting follow-up patient care will result in cancellations. There are no guaranteed hours with this contract. Clinicians must achieve weekly productivity points. Adjunct opportunities may arise at nearby care centers if patient loads are low.
