Job Summary

Join our centralized ambulatory pharmacy team where your expertise makes a meaningful impact. As a Clinical Staff Pharmacist, you’ll ensure safe and effective medication use by reviewing prescriptions, maintaining accurate patient profiles, and checking for drug interactions, duplications, and allergies. You’ll collaborate closely with specialty clinics, specialty pharmacy, and medication access teams—prior experience in specialty pharmacy is preferred. A key part of your role will involve working within the Multi-Step Order Transmittal (MSOT) queue, where precision and efficiency are essential. You’ll also evaluate prior authorization requests, interpret clinical data, and work with healthcare providers to support coverage decisions and the appeals process. Beyond daily tasks, you’ll contribute to initiatives that improve care coordination and patient outcomes.

Experience and Education

One (1) year with experience in a hospital setting required for an inpatient position; 1 year retail experience for retail position preferred.

Bachelor of Science (BS) degree or Doctor of Pharmacy (PharmD) degree required.

Eligible for licensure by Texas State Board of Pharmacy (TSBP) required and Texas license with TSBP within 30 days of date of hire.

Sterile compounding (IV) certification accredited by ACPE.

Basic Life Support (BLS) certification with course accredited by the American Heart Association (AHA) or American Red Cross (ARC) required by start date.

Advanced Cardiac Life Support (ACLS) certification accredited by AHA or ARC required for inpatient positions prior to or within 6 months of employment.

Job Duties – Essential/Primary

Performs job responsibilities in accordance with organizational and departmental policies, procedures, performance improvement objectives and environmental/infection control standards, as well as state and federal laws and regulations.

Performs all computer order entry in an accurate, thorough and timely manner.

Reviews/updates medication profiles (therapies, allergies, height, weight, etc.) and is cognizant of patient charges.

Reviews medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety.

Seeks clarification if necessary and documents interventions (e.g., ADR).

Works multi-tasking and problem solves independently and effectively with minimal supervision.

Takes charge when necessary and provides direction; uses good judgement.

Is cross trained and maintains competency in multiple areas.

Enforces the formulary system and follows policy when acquisition of non-formulary product is necessary.

Participates in cost-effective use and selection of pharmaceuticals.

Communicates drug information to physicians, nurses, etc., in a courteous and professional manner.

Assists in patient/family education and counseling regarding drug therapy and side effects.

Documents clinical activities clearly, demonstrating cost savings and providing information for follow-up by fellow pharmacists.

Strives to improve patient satisfaction, as measured by Press Ganey scores.

Additional Duties

Manages technicians and all other supportive personnel.

Delegates appropriate tasks and validates accuracy of medications distributed from the IV lab, central pharmacy or decentralized automated dispensing machines.

Audits and signs out all controlled substances in accordance with state/federal guidelines and organizational policies.

Investigates and corrects discrepancies and unusual usage patterns/trends.

Participates in educational programs, in-services and meetings; participates in new employee orientation and training; takes on additional projects as needed.

Participates in the clinical program and supports P&T initiatives by monitoring target drugs for protocol discontinue, substitution, IV to PO, etc.; collects data/measures and communicates opportunities for improvement.

Performs therapeutic monitoring of specified drug therapies and recommends dosage adjustments as needed based on renal function evaluation and pharmacokinetic calculations.

Other duties as assigned.

Core functions may include direct patient care, handling patient records, and related activities as applicable.

Equivalents in lieu of stated minimum require prior approval. This position is security-sensitive and subject to Texas Education Code 51.215, which authorizes UT Southwestern to obtain criminal history record information.
