Overview

The Care Manager/“Designated Care Manager” is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. The Care Manager demonstrates Sunrise Senior Living’s Mission to champion quality of life for all seniors in accordance with laws and Sunrise standards, policies, and procedures. Base pay range

$18.75/hr - $23.45/hr Responsibilities

Resident Care: Review, read, notate, and initial Daily Log to document and learn about pertinent information about residents; participate in ISP development and updates; monitor designated residents during shifts; observe and document changes in residents’ condition; respond to call bells; greet guests and residents; assist with continence management; communicate with families; support residents in maintaining independence and safety; assist with activities of daily living (ADLs) and encourage resident participation per ISP; engage residents in life skills and reminiscence activities; maintain a warm, calm environment and adhere to Resident Rights and Sunrise Principles of Service. Risk Management and General Safety: Ensure compliance with occupational health and safety regulations; practice PPE and other safety procedures; report accidents and unsafe conditions; keep hazards clear of exits and follow proper handling of hazardous materials. Housekeeping and Laundry Services: Maintain resident rooms and common areas in a clean condition; handle resident laundry and dining linens as needed; complete assigned housekeeping tasks. Dining Service: Serve meals, assist with dining room setup/cleanup, follow dietary guidelines per ISP, observe changes in dining habits, and provide room service as needed. Reminiscence Neighborhood: Implement life skills and enrichment activities (IEA) and seasonal programming; integrate residents’ life skills into daily routines; coordinate family engagement through monthly letters as appropriate. Training and Team Success: Participate as a team member, uphold Team Member Credo and Principles of Service; engage in engagement programs and training; attend required meetings and training; comply with infection control and regulatory requirements. Core Competencies: Remain calm under pressure, manage multiple priorities, communicate effectively, demonstrate good judgment and problem solving, and maintain organizational skills. Experience and Qualifications

High School diploma/GED (as required by state/provincial regulations). CPR and First Aid as required by regulations. Must be at least 18 years of age. Previous experience working with seniors preferred; desire to serve and care for seniors. Ability to make decisions in the resident’s best interest; if applicable, driver duties per Sunrise requirements. Benefits and compensation
