The Care Manager/"Designated Care Manager" provides the highest quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager delivers hands-on care and supports residents physically and emotionally as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe, home-like environment. Responsibilities & Qualifications
Essential Duties As a part of the Sunrise team, support the Mission, Principles of Service and Core Values. Resident Care: Review, read, notate, and initial Daily Log; participate in ISP development and updates; review designated assignments; be responsible for a designated group of residents during the shift; observe, report, and document changes in residents’ conditions; attend daily Cross Over meetings; notify supervisor of changes; respond to call bells; greet guests and residents; handle phone calls; assist with continence management and sanitation; communicate with families; help residents maintain independence and safety; assist with ADLs and Encourage/Assist (IEA) residents per ISP; engage residents in life skills and reminiscence activities; maintain warmth and a calm environment; follow safety regulations and infection control practices; host daily activities. Risk Management and General Safety : Partner with the team to ensure compliance with safety regulations, use PPE, report accidents/incidents, address hazards, understand exposure protocols for bloodborne pathogens and hazardous materials, and ensure safe storage of oxygen. Housekeeping and Laundry Services : Maintain and clean resident rooms, do laundry per ISP, manage dining room linens, complete assigned housekeeping tasks, keep common areas clean. Dining Service : Serve meals, promote a pleasant dining experience, assist with setup and cleanup, participate in pre-meal meetings, follow ISP dietary instructions, document dining changes, provide room service as needed, practice safe food handling. Reminiscence Neighborhood : Engage residents in life skills, attend social activities, incorporate "Creating Pleasant Days", use residents’ life skills in daily routines, provide multi-sensory experiences, monthly letters home with residents and families. Training & Team Contribution : Participate as a team member, uphold Team Member Credo and Principles of Service, contribute to engagement programs and surveys, attend meetings and trainings, comply with infection control and regulatory requirements, maintain required trainings, perform other duties as assigned. Core Competencies : Remain calm in difficult situations, handle multiple priorities, communicate effectively, strong organizational/time management, good judgment and problem-solving. Experience & Qualifications : High School diploma/GED; CPR and First Aid as required by regulations; must be at least 18; prior experience with seniors preferred; desire to serve seniors; ability to act in residents’ best interest; for drivers, sign Driver job description and safety duties where applicable.
