Overview
The Care Manager/"Designated Care Manager" is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. The role demonstrates the Sunrise Mission: to champion quality of life for all seniors, in accordance with laws, Sunrise standards, policies, and procedures to promote high-quality care.
Responsibilities
Resident Care
Review, read, note, and initial Daily Log to document and learn about pertinent information about residents.
Participate in the development of ISPs and monthly updates.
Review designated assignments and supervise a designated group of residents during the shift.
Observe, report, and document symptoms and conditions of residents and changes in condition (e.g., skin, behavior, alertness, weight, diet, activity participation).
Attend daily Cross Over meetings with the lead care manager.
Notify supervisor and resident care director of increased care needs; inform supervisor of any resident changes in condition.
Respond to security system and resident call bells promptly; take appropriate action including resetting bells.
Greet guests, family members, residents, and team members; answer and respond to phone calls professionally.
Assist with continence management and sanitation of resident suites and restrooms per standard procedures.
Communicate with families and serve as a resource as needed; support resident independence and safety with dignity per Resident Rights and Sunrise Principles of Service.
Assist/resident participation in activities of daily living (ADLs) and encourage IEA participation per ISP; engage residents in life skills and reminiscence activities.
Maintain a warm, calm environment while following safety regulations and infection control practices.
Engage in daily activities with residents and monitor changes in dining and overall well-being.
Risk Management and General Safety
Ensure the community complies with health and safety regulations; promote risk management programs and policies.
Follow safety procedures including use of PPE, fire safety, SDS, and lockout/tagout procedures.
Record and report accidents, unsafe conditions, and hazardous conditions immediately; keep exits accessible and free of hazards.
Understand proper handling and disposal of bloodborne pathogens, infectious waste, and hazardous materials; report exposures as required.
Housekeeping and Laundry Services
Maintain and clean resident rooms; manage laundry as ISP dictates; handle dining room linens and napkins.
Complete assigned housekeeping tasks; keep common areas clean and tidy.
Dining Service
Serve meals in the dining room and assist with dining room setup and cleanup as assigned.
Support a pleasant dining experience, follow diets per ISP/Confidential Diet Board, and document changes in dining habits.
Provide room service as needed; ensure safe food handling in accordance with care standards.
Reminiscence Neighborhood Care Manager Responsibilities
Encourage engagement in life skills and daily routines; support multi-sensory experiences and monthly family communications.
Training and Team Contribution
Participate as a team member; uphold Sunrise Credo and Principles of Service.
Contribute to engagement programs and attend required meetings and trainings; maintain compliance with infection control and regulatory requirements.
Perform other duties as assigned.
Core Competencies
Ability to stay calm in difficult situations; manage multiple priorities.
Strong written and verbal communication; good judgment and problem-solving skills.
Experience and Qualifications
High School diploma/GED (as required by regulations).
CPR and First Aid as required by regulations.
Must be at least 18 years old; prior experience with seniors preferred.
Ability to act in the resident's best interest; driver role may require additional duties per policy.
