Physician – Director for Home‑Based Primary Care (HBPC)

Summary Lead the HBPC program, provide clinical leadership, oversee patient care in the home setting, and manage the HBPC medical director responsibilities.

Major Responsibilities

Lead and direct the development of the HBPC program.

Ensure program compliance with accreditation standards (VHA, Joint Commission).

Provide clinical leadership and program management for HBPC.

Assume primary medical responsibility for HBPC patients, managing medical problems and coordinating care.

Act as a consulting physician for advanced practice nurses and as a backup for other HBPC providers.

Provide geriatric and palliative care consultation in acute, outpatient, and home settings.

Collaborate with community home‑health and hospice agencies.

Qualifications

U.S. citizenship (non‑citizens only if no qualified citizen available).

Doctor of Medicine or equivalent, from an accredited U.S. or foreign institution.

Current full, unrestricted license to practice medicine in a U.S. state or territory.

Completed residency training in an accredited core specialty leading to board certification (or equivalent).

Board certified or board eligible in Geriatrics or Palliative Care (preferred).

Proficiency in written and spoken English.

Ability to pass background/security investigation and pre‑employment physical.

Employment Details Work schedule: 7:00 am – 3:30 pm / 7:30 am – 4:00 pm / 8:00 am – 4:30 pm, Monday‑Friday. Competitive salary, annual performance bonus, paid time off 50‑55 days per year, traditional federal pension, 401K match up to 5%.
